Important: Half-life is not the same as how long a person feels an effect. Sedation and anxiety relief may wear off before the medication and its metabolites have left the body. Age, liver function, dose, frequency of use and other drugs can substantially change both effects and clearance.

How do diazepam and lorazepam work?

Both medications increase the effect of gamma-aminobutyric acid, or GABA, at the GABA-A receptor. GABA is an inhibitory neurotransmitter that reduces neuronal activity. This mechanism can produce anti-anxiety, sedative, anticonvulsant and muscle-relaxing effects.

Because both drugs act on the same receptor system, they share many benefits and risks. However, they are not interchangeable milligram for milligram. Their absorption, redistribution, metabolism, active metabolites and approved formulations differ.

What are Valium and lorazepam approved to treat?

The approved use depends on the specific product and route of administration. Claims about oral tablets should not be mixed with indications for injectable or rectal formulations.

Diazepam oral tablets

The U.S. prescribing information for oral diazepam includes:

  • management of anxiety disorders or short-term relief of anxiety symptoms;
  • symptomatic relief of acute agitation, tremor, impending or acute delirium tremens and hallucinosis during acute alcohol withdrawal;
  • adjunctive relief of certain skeletal muscle spasms and spasticity; and
  • adjunctive use in convulsive disorders, not as sole therapy.

Lorazepam oral tablets

The U.S. prescribing information for oral lorazepam includes management of anxiety disorders and short-term relief of anxiety symptoms or anxiety associated with depressive symptoms. The label states that anxiety related to everyday stress usually does not require treatment with an anxiolytic and that effectiveness beyond four months has not been systematically assessed.

Lorazepam injection has additional acute-care indications, including status epilepticus and preanesthetic use. Those indications should not be presented as though they automatically apply to oral lorazepam tablets.

Which works faster, and which lasts longer?

Diazepam reaches peak blood concentrations sooner after an oral dose, on average, but a patient’s perceived onset can vary. Diazepam also remains in the body substantially longer because the parent drug and its active metabolites are eliminated slowly.

According to U.S. labeling, fasting oral diazepam reaches peak plasma concentrations in an average of 1 to 1.5 hours, with a reported range of 0.25 to 2.5 hours. Oral lorazepam reaches peak concentrations at about 2 hours. Food, formulation and patient-specific factors may alter these timings.

Diazepam’s prolonged terminal half-life and long-lived active metabolite can lead to accumulation with repeated dosing. This may be clinically useful in some settings, but it can also contribute to next-day sedation, impaired coordination and falls, particularly in older adults or people with impaired hepatic clearance.

Is 1 mg of lorazepam equal to 10 mg of Valium?

Some clinical conversion references estimate that 1 mg of lorazepam is approximately equivalent to 10 mg of diazepam. However, benzodiazepine equivalence is inexact, and authoritative tables do not all agree.

The 2025 Joint Clinical Practice Guideline on Benzodiazepine Tapering compares multiple published estimates. Depending on the source, 10 mg of diazepam may correspond to approximately 1 to 2 mg of lorazepam. The guideline emphasizes that conversion is patient-specific and should account for factors such as age, metabolism, other medications and the clinical response during a gradual transition.

Clinical pearl: A conversion table is a starting estimate, not a prescription. A clinician may need to adjust a cross-taper based on withdrawal symptoms, sedation and the patient’s response. Patients should never calculate and perform their own benzodiazepine substitution.

How do their metabolism and drug interactions differ?

Diazepam has more potential for metabolism-related interactions because it depends substantially on cytochrome P450 enzymes. Lorazepam undergoes direct glucuronidation, but it can still have serious pharmacodynamic interactions with other sedating substances.

Diazepam metabolism

Diazepam is metabolized through CYP3A4 and CYP2C19 to active metabolites, including N-desmethyldiazepam, temazepam and oxazepam. Drugs that inhibit these enzymes may increase or prolong diazepam exposure and sedation. The diazepam label identifies examples including ketoconazole, fluvoxamine, fluoxetine, omeprazole and cimetidine.

Lorazepam metabolism

Lorazepam is conjugated in the liver to lorazepam-glucuronide, which has no demonstrable central nervous system activity, and is then excreted mainly in the urine. This pathway can make lorazepam less vulnerable to CYP-mediated interactions, but “fewer CYP interactions” does not mean “no interactions” or “safe for everyone.”

What side effects do Valium and lorazepam share?

Both can cause dose-related central nervous system depression. Common or clinically important effects include:

  • drowsiness and fatigue;
  • dizziness;
  • muscle weakness;
  • poor coordination or ataxia;
  • confusion or memory impairment;
  • slowed reaction time;
  • falls and fractures, especially in older adults; and
  • paradoxical agitation, irritability or disinhibition in some patients.

Risk can increase with higher doses, longer exposure, older age, respiratory illness, hepatic or renal impairment and concurrent sedating medications. Can Valium or lorazepam cause addiction and withdrawal?

Yes. Both medications can lead to physical dependence, and both carry risks of misuse, addiction and withdrawal. Physical dependence is an expected neuroadaptation that can occur during prescribed use; it is not identical to a substance use disorder, although the two can coexist.
